We are committed to providing effective communication support for people with disabilities and individuals whose primary language is not English. Discrimination complaints must be submitted within sixty (60) days of the incident. Complaints should include details of the issue and the resolution sought. Serenity Hospice Care will provide a written decision based on the preponderance of evidence within thirty (30) days of the complaint. If you wish to appeal the decision, please submit a written appeal to us within fifteen (15) days of receiving the decision. We will respond in writing within thirty (30) days of receiving the appeal.

Serenity Hospice Care will not retaliate against anyone who opposes discrimination, files a discrimination complaint, or participates in an investigation of such a complaint. You are encouraged to exercise your rights under the law, and our procedures are designed to ensure you may do so safely.
