Serenity Hospice Care complies with applicable federal civil rights laws and does not discriminate, exclude, or treat people differently based on social status, political beliefs, sexual orientation, race, color, religion, national origin, age, sex, or disability in relation to admission, access to treatment, or employment.
We are committed to providing effective communication support for people with disabilities and individuals whose primary language is not English. This includes offering free aids and services such as qualified sign language interpreters, accessible written materials (large print, audio, or electronic formats), and qualified interpreters and translated information for non-English speakers. If you believe Serenity Hospice Care has not provided these services or has discriminated in any other way, you may file a complaint with us in person, by mail, or by phone.
Discrimination complaints must be submitted within sixty (60) days of the incident. Complaints should include details of the issue and the resolution sought. Serenity Hospice Care will provide a written decision based on the preponderance of evidence within thirty (30) days of the complaint. If you wish to appeal the decision, please submit a written appeal to us within fifteen (15) days of receiving the decision. We will respond in writing within thirty (30) days of receiving the appeal.
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, through one of the following methods:

Serenity Hospice Care will not retaliate against anyone who opposes discrimination, files a discrimination complaint, or participates in an investigation of such a complaint. You are encouraged to exercise your rights under the law, and our procedures are designed to ensure you may do so safely.
